Assistant Coaches are not being required to submit these, only head coaches.  There are three options: 1.) Give us the physical forms when picking up your uniforms (and we will mail payment for you)  2.) Bring us a printed out form if you have submitted these online  3.) If you have already received these clearances, just bring us a copy when you pick up your uniform.  You can print them out or submit online at the following links:
